Job Summary

OT Home Health - 40HRS DAYS
START DATE: ASAP START, ORIENTATION EVERY MONDAY

Certs Required: BLS
Documentation Systems Required: Pointcare/HCHB/ OASIS E

Patient Age Group: Adults, Geriatrics
Daily Patient Load: 6 points per day
Productivity Standards: 30 points per week

Skills Required: Home Health Care, Amputee, Arthritis program, CVA Rehabilitation, Dementia/Alzheimer's, Hand Injury, Traumatic Brain injury, Lower Extremity, Multiple Sclerosis, Nerve Injury, Parkinson's disease, Spinal cord injury, Tendon Repair, Upper Extremity, Adaptive Equipment, Community Re-entry, Home Assessment, Pre-Discharge Planning, Wheelchair Evaluation and Positioning, Hip Fracture and Total Hip Replacement, Knee Injuries and Total Knee Replacement, Standardized Assessment, Infection Prevention/Fall/Risk Assessment/chronic disorder/functional activities

Professional Skills and Knowledge Required: Lifting and transfer, Patient/Family Education, Safety and injury prevention

Billing/Documentation Required: Assessment, Discharge, Evaluation, OASIS, Plan of Care, Reassessments

Skills Preferred: National Patient Safety Goals/ Core Measures/Acute disorder, Adult Outpatient, Adult Rehabilitation Facility, LTAC (Long Term Acute Care), Pediatric Rehabilitation Facility, Skilled Nursing (SNF), Coma Management, Wheelchair Evaluation & Positioning, Hand Injury, Soft Tissue Injuries, knees prosthetics, Dynamic, Functional, and Static splints, Crisis Intervention, Substance Abuse, joint mobilization, Cognitive Assessment, Other Perceptual Assessment

Scrubs not provided: Navy pants, white or navy top

• Two (2) Supervisor references required
• Return Staff Policy If a nurse has held previous employment with Medstar, must be gone a minimum of 1 year to return as a traveler.
